So how do you know if a herniated disc will require surgery? 90% of herniated discs respond very well to conservative management and treatment, without surgery. Some of them may take a while but a surgical case would be the one that does not respond to conservative management, does not respond to non-surgical alternatives and these are the patients that fail conservative management. The quality of they life is affected so much that they cannot function. Those are the cases that will require surgery for a disc herniation. Dr. Eingorn has seen patients in this unfortunate situation. There was a case few months ago where a patient came all the way from Iowa, and he has been treated for months with physical therapy and chiropractic care. His disc was beyond natural repair. Part of the disc was lodged in the neural frame where the nerve root sits. It is like having a pebble in your shoe. Unless its removed no matter how much you wiggle, it is not going to help. If you loose bowl or bladder control, those are typically the surgical cases. What is a herniated disc? Disc herniation occurs when the spongy discs surrounding the vertebrae of the spine become damaged leading to bulges and/or ruptures. While herniated discs do not necessarily cause symptoms, they are often quite painful. Some of the most common symptoms associated with disc herniation include weakness, numbness, and pain in the lower back, buttocks, and legs. Non-surgical treatments for disc herniation In most cases, herniations will resolve on their own or with the help of non surgical treatments, such as non surgical lumbar disc decompression or herniated discs chiropractic treatments. Typically a few months of treatment will resolve the issue without the need for invasive surgery. Non surgical lumbar disc decompression and herniated discs chiropractic treatments are both considered quite effective and can often be used in conjunction with pain medications and physical therapy to achieve the desired results. Dr. Eingorn, like many physicians, also focuses on helping patients to develop healthy diet and exercise regimens to achieve and maintain a healthy spine. Usually surgery will not be considered if improvements are seen within four weeks of the start of treatment. Patients of Dr. Eingorn are fortunate in that his expertise in non-surgical treatments is well known, and many of his patients have reported fast, long-lasting results. The healing of a herniation requires the material within the disc to be resorbed thus relieving the pain. Relatively fast results usually indicate that the body will be willing and eager to heal without surgical treatment. When will surgery be considered? Surgical herniated disc treatments may be recommended by Dr. Eingorn if you are experiencing significant pain that is interfering with your daily life and activities or if your symptoms do not begin to improve after four weeks. If your symptoms continue to worsen throughout your treatment or you're in need of quicker results, surgery may be the best option for you. Additionally, if you're willing and able to go through the lengthy rehabilitation process following your surgery, you may be able to get the results you seek quicker through surgical methods. In this video Dr. Eingorn discusses conservative treatment for herniated or bulging discs. Conservative treatment for disc herniations can vary greatly. It will depend on where the disc is, whether it is in the neck, the lower back and whether it is in the thoracic spine. These will be very very different treatments. Usually it will require some type of traction and compression. It will require some type of physiotherapy, like electrical stimulation, it might require icing. It could require mobilization of the spine to allow for the disc to start healing. Once the patient is out of acute pain and out of danger, it will require sometime of exercise protocol to help them strengthen the core muscles that support that area. There are a lot of nutritional considerations. Discs are 90% water and if patients don’t drink enough water the discs are not going to recover. Simple things like this make a huge difference. SO treatments may vary greatly, even though I am a chiropractor, my first line of defense is chiropractic care, both traditional and alternative. In this video Dr. Alex Eingorn of Better Health Chiropractic in New York City, describes a herniated disc as he often does with patients when they first arrive at his practice with herniated discs. Herniated discs are very common and we see a lot of patients with disc herniations and most people don’t understand right away what it means. Discs are cushions that basically cushion the peace between the vertebrae in the spine. They allow us to have free movement and they allow us to absorb shock and they also prevent the vertebrae and joints from collapsing into each other. They also allow space for the nerve roots to feed and collect information from the brain. Here is a model of a normal disc for you to see. This is the normal disc that is similar to a jelly donut. It has a liquid, a gelatinous substance inside called the “nucleus pulposus” and a very tough but flexible outer ring called the “annulus fibrosus”. Here is the spinal cord right here behind the disc and the nerve roots are right here. So it is a very sensitive structure. Now herniated discs can happen in the neck, in the mid back, which is rather rare but it can happen sometimes, and they happen in the lower back. Most of the time we will see a patient with a disc herniation in the lower back, that is the most common. Since there are many levels of vertebrae, the problems can occur at many levels. The symptoms also can vary depending on the type of disc herniation and where it is in the body. Is it possible that you have a disc herniation and you don't know it? Not experiencing any pain? You may still be living with a herniated disc! Herniated discs are widely known as highly painful and for good reason. Many people experience significant pain that can make usual activity difficult or even impossible. While this is certainly considered the norm, believe it or not, there are some individuals who experience disc herniation without any symptoms at all. There are a number of reasons why herniated disc symptoms may not be present in a particular individual. For one, the immune system of the body is sometimes able to identify and attack the herniation as it would a foreign material. This leads to the herniation being diminished in size and impacting the nerve root far less or not at all. The size of the herniation can also decrease when the water within is absorbed back into the body reducing the affect on nearby nerves. There are also those that for reasons unknown simply do not feel disc herniation. Recent MRI studies have indicated that there are some people who, despite having a large herniation, do not experience pain. A wide spinal canal may be the reason for this as the wider area may allow for a larger herniation that is not necessarily accompanied by low back pain. While the absence of the usual symptoms certainly does not mean that a herniation does not exist, pain-free herniated discs typically have minimal inflammation. This makes herniated disc treatment often easier to accomplish without the need for surgery. Fortunately, even for those individuals who are not so lucky as to experience no pain, there are many treatment options available that may negate the need for invasive procedures. Common symptoms of a herniated disc When a disc herniates, the outer layer of the disc ruptures resulting in the bulging of the nucleus pulposus, the gel-like center of the disc. This material can then cause the pinching or compression of nearby nerve sacs. The herniated gel contains proteins that can lead to inflammation that can then impact the nerves. The result is often sciatica within the lower back which can range from mild to severe. The most common symptoms of herniated discs include: - leg and arm pain - numbness - weakness While this list of potential symptoms is certainly not exhaustive, the above symptoms can often indicate an existing disc herniation. However, if you're experiencing any symptoms that you believe may indicate a problem, do not hesitate to contact Dr. Eingorn to receive a consultation. Non-surgical treatment options for herniated discs Fortunately for those who are living with herniated discs, there are a number of treatment options available. Non-surgical options are the first course of action in most cases and, in many situations, surgery ultimately proves to be unnecessary. A couple of the most effective, highly recommended non-surgical options include: Spinal injections for herniated discs: steroid injections can be used in order to reduce inflammation in the spine and alleviate its impact on nearby nerves. Non-surgical lumbar disc decompression: certain chiropractors and physicians including Dr. Eingorn utilize this computerized decompression technique as a means of manipulating the spine and reducing pressure on the nerves. This type of therapy is known for being highly effective at reducing nerve compression issues. While early intervention involving surgery may be necessary in those instances where severe pain and/or disability are present, treatments such as these are often recommended first and can be very effective on their own. In this video Dr. Eingorn explains what the initial visit to his practice is like. Generally when the patient comes to us for the first time, we will have them fill out a questionnaire that is pretty extensive. It has many questions about their history and their previous health problems. Then I will sit down with them and we will have an interview. The Interview Predominantly what we are going to do first is we are going to focus on the problem that they are presenting with. “Doc I have neck pain.” “Doc I have lower back pain.” “Doc my leg does not work.” “I am having migraines.” Then I will ask them more questions about the problem. When did the problem start? How did it start? Were there any extenuating circumstances? Was there an accident? Was there an issue? Was there an incident of some kind? We want to get as full of a picture as possible on the person, on the patient, not just on their pain. After that we will commence with the examination. The Examination The examination will consist of pretty much all the standard physical tests, blood pressure, vital signs and then we will move on to orthopedic and neurological exam. Generally, we will first focus on the immediate problem. We will check the range of motion. We will check if they have any sensitivity, any illness or any limitations. We will check how sensitive they are to pain. Clinical Decision After we have done the thorough exam then we will have a clinical decision making process to make. Usually it is done with the patient, sometimes we want to just go ahead and then render conservative treatment. Sometimes we want to get some imaging, whether it is an X-ray MRI or MRA or some type of other tests we need to have done. Sometimes we send them out for blood tests. Usually we will get in touch with a primary care physician to get more of a history and to keep in touch to make sure that we are all on the same page. Once those decisions are made then of course the treatment plan is discussed with the patient and we commence with the treatment. A herniated disc refers to an issue of the spine in which the interior portion of a spinal disc pushes outward through a rupture in the disc's exterior. Herniations can lead to a number of problems including irritation of nearby nerves, numbness, weakness, and even extreme pain. However, in some cases, symptoms of the herniation do not occur leading the issue to go uncorrected. How can I tell if I have a herniated disc? It's possible to be living with a disc herniation without knowing it although this is hardly common. There are a number of herniated discs symptoms you might experience, some of which can be quite debilitating and painful. The most likely herniated discs symptoms include: Leg and arm pain: If the disc is located in the lower back, the pain will typically be located primarily within the calf, thigh, buttocks and the feet. When in the neck, herniations can cause the most intense pain within the arms and shoulders. Radiating pain is also quite common. Tingling or numbness Weakness: Muscles in connection with the affected nerves will likely weaken after a herniation. What causes disc herniation? In most cases, disc herniation is caused by gradual wear and degeneration of the discs. As the spine ages, the discs will tend to lose water from their interior. This leads to reduced flexibility and an increased likelihood of rupture. In some cases, even a simple twist of the body can result in a herniation. While a single physical move can result in an immediate herniation, it's unlikely that a person will be able to identify the exact time the problem occurred. Falls, blows, or simply using your back instead of your legs to lift a heavy object can be the culprit for a herniation. How is a herniation diagnosed? In order to diagnose the problem, the physician will ask you some questions about the pain. For example, is it low back pain or another type of discomort? The provider may ask you to lay flat and to move your legs, arms, or shoulders in order to identify the source of the pain. While a physical exam is typically all that is needed, your doctor may also require imaging techniques, such as X-rays, CT scans, and MRIs. What are the treatments used to treat the problem? The types of herniated disc treatments used will depend on whether the patient is dealing with low back pain or another type of issue. In some cases, conservative treatment is all that is required. Simply avoiding certain positions and following recommended exercise regimens can help to relieve the pain. Over-the-counter and prescription medications may also be used. For mild to moderate pain, ibuprofen or naproxen may be sufficient while severe pain may warrant the use of narcotics such as oxycodone or nerve pain medications such as tramadol. Depending upon the patient, the provider may also recommend spinal injections for herniated discs. Cortisone or other types of spinal injections for herniated discs may prove helpful at reducing inflammation and relieving pain. In some cases, muscle relaxants may also be recommended as a treatment option. Certain non-invasive procedures may also be recommended and used by the provider. Herniated discs chiropractic treatment and non surgical lumbar disc compression (non-surgical lumbar disc decompression) have shown great promise at treating herniation. These types of therapy serve as an alternative to surgeries which are typically only used as a last resort when the pain and the herniation is not resolved by using other methods. Both non surgical lumbar disc compression (non-surgical lumbar disc decompression) and herniated discs chiropractic treatment are therapies that are personalized in order to help each particular patient. In fact, the former is a type of motorized traction that can be customized both before and during the procedure to give the patient the most benefit.
